Insurance Program, Long-term Care Insurance Program and National
Pension Plan
National Health Insurance Program
In Japan, everyone is required to join either the National Health
Insurance Program or the Workers’ Health Insurance Program.
The Workers’ Health Insurance Program is a plan whereby employees
are insured through their employers. Foreign residents who plan
on staying in Japan for one year or longer are also required to
join the National Health Insurance Program. When you join the program,
you will receive a National Health Insurance Certificate and must
pay a premium. Once you join, you can only withdraw under certain
conditions.

When to Join, When to Withdraw and Other Situations
In the following cases, report to the city office for the necessary
procedures with your foreign residents’ registration card
and National Health Insurance Certificate within 14 days.
To Join
(1) When moving into Chiyoda City, including when coming directly
from abroad.
(2) When you withdraw from your workers’ health insurance
policy.
(3) When a baby is born.
(4) When you are no longer covered by the social welfare program.
To Withdraw
(1) When you move out of Chiyoda City (or leave Japan).
(2) When you join your workers’ health insurance program.
(3) When a person who is covered by your policy dies.
(4) When you become a recipient of the social welfare program.
Others
(1) Report to the city office when you change your residence within
Chiyoda City or when changes are made in your foreign residents’
registration card such as the name and the head of household.
(2) Report to the city office when you have lost your National Health
Insurance Certificate.
Premiums and Benefits
Premiums
The premium must be paid starting from the month you become insured.
As soon as you move into Chiyoda City, or your membership in another
health insurance system terminates and you join the National Health
Insurance Plan, you become liable to pay the premium.
If you are between 40 and 64 years of age and enrolled in National
Health Insurance (Type 2 Long-term Care Insurance Member), the head
of the household must pay a combined National Health Insurance premium
that consists of the basic medical portion plus long-term care insurance.
If you are 65 or older (Type 1 Long-term Care Insurance Member),
you must pay a separate Long-term Care Insurance premium in addition
to your National Health Insurance premium. (Usually, deducted from
your pension.)
How the premium is calculated
The premium is calculated on a yearly basis (April - March) based
on the insured’s amount of resident tax and number of dependents.
Basic share Total of income-derived amount based on the resident
tax amount which is calculated on income
plus per capita amount which is imposed on every member .
The maximum amount is ¥590,000.
Nursing share Total of income derived amount based on the resident
tax amount of people who are 40 to 64 years of age
plus per capita amount which is imposed on every member in this
age bracket .
The maximum amount is ¥90,000.
The National Health Insurance premium is the total of the basic
share plus the nursing share. The head of the household pays the
total amount divided into10 payments.

Benefits
When you submit your National Health Insurance Certificate to a
medical facility for treatment, you only need to pay 30% of the
medical costs (those applicable). When an insured person gives birth
or dies or when high nursing care costs are incurred, specified
amounts of benefits are provided. For more information, contact

Long-term Care Insurance
Category 1 Member (Those 65 years of age or older)
All foreign residents who are 65 years of age or older, have completed
foreign resident registration and have lived in Japan for one year
or longer are eligible for membership. Those applicable must apply

Applying for Approval
Those who need long-term care services should, first of all, apply
for approval of their need. Those who are approved as “Requires
Assistance/Requires Nursing Care,” should then develop their
own nursing service plan; approved members may take advantage of
nursing services based on their service plan. As a rule, 10% of
the fees must be personally borne.
Category 2 Member (Those between 40 and 64 years
of age)
All those who are enrolled in a public health insurance plan are
eligible for membership in the Long-term Care Insurance Plan.
Applying for Approval

caused by aging, should apply for approval of their need. Those
who are approved as “Requires Assistance/Requires Nursing
Care,” should then develop their own nursing service plan;
approved members may take advantage of nursing services based on
their service plan. As a rule, 10% of the fees must be personally

National Pension Plan
All those living in Japan, including foreign residents, who are
20 years old or older but under 60 and who are not enrolled in either
the Public Welfare Pension Plan or the Mutual Aid Association Pension
Plan must enroll in the National Pension Plan. As a general rule,
with payment of premiums for 25 years or more within the ages mentioned
above, you will be eligible to receive benefits from the Senior
Citizen Basic Pension beginning at age 65. When a foreign resident
who has paid premiums for a total of six months or longer (adding
the number of months of payment of premiums and one-half the number
of months of 50% exemption) returns to their home country without
receiving a pension, he or she may receive a lump-sum withdrawal
benefit if they apply for it within two years of their return to
their home country.
